If you are starting an account with a new Provider, in addition to completing and submitting a Salary Reduction Agreement form to Envoy Plan Services, you must open an account with the Provider of your choice by contacting that Provider. Please submit any paperwork the Provider may require directly to that Provider.

Your salary reduction cannot take place until your account is opened with your selected Provider.
